My dear mother lived in a town about 3 hours from me. She had been admitted because when she fell (no broken hip) they found a “spot” on her brain on her MRI. She was never seen by a specialist. Just admitted to a local hospital. I left my home with my wife, traveled to the hospital and found her in her room alone-unconsious and getting no care—absolutely none. I asked the nurse if the doctor had made rounds that day and if he was aware of her condition.She said he had not been by not since her admission. It was late at night when I arrived and she had been unconscious for hours and no one had seen her. No one.
I called the doctor and chewed him out. I told him it was the worse medical care I had ever seen and I was transferring her to my hospital miles away—right then. I then called a neurosurgery friend of mine and had her transferred. I was told by her doctor in the first hospital that she would be dead in a short amount of time. My neurosurgery friend, in my hospital, operated on her, she woke up post-op and lived over a year, perfectly well and celebrated her 83rd birthday with us and her sisters many months later.
With the arrival of AI and medical protocols, this will only get worse, much worse. The protocols will be written by academic doctors in medical centers—under the control of pharmaceutical companies. These protocols will be enforced in all hospitals. Most physicians, especially non-surgeons, will be replaced by “protocols”. Hired, non-physicians, will administer the protocols. As in the COVID-19 psychops, they will be absolutely enforced.
As many suspect, these protocols are designed purely to save the government money. The poor, the elderly, the welfare client, the deformed, the chronically ill and people whose illness is costing the “state” money, are scheduled to be among the ones chose to die. The medical University hospital also murdered my youngest grandchild. I fought them tooth and nail, to no effect. They gave him a week to live. I remember during my graduate study at Duke in 2002, one of my professors talked about the aging population and asked what would happen around 2020 to deal with the foreseeable economic "burden" it would create.

For me, this is a really complicated issue. My hard stop is that the state should have no role in deciding if or when to terminate someone's life. But I don't necessarily believe that all doctor-assisted suicide is wrong.
Earlier in my lifetime, families were close, the long arm of the law wasn't so long, and doctors had long-term relationships with the whole family. My ex-wife tells a story of her grandmother's death in 1968, assisted by the family doctor, with everyone agreeing, but no one saying aloud "up her dose of morphine". The patient was in terrible pain, and when her daughter asked the doctor, "Isn't there something that you can do?" the doctor knew exactly what she meant.
Six years ago, I was invited to the bedside of a long-time friend with the message that he was dying. What I didn't realize was that his half-brother -- his closest living relative -- had made the decision to sedate him until his breathing stopped in front of a small gathering of friends and family. I watched, appalled, and feeling dislike for the half-brother who needed money and was my friend's heir. But I decided not to say anything because (1) I remembered that my friend had spoken about not wanting to prolong his own death, and (2) I had watched his quality of life go from vibrant and active to confined and empty over the course of about 5 years.
To me, this story meant two things: First that there is a danger in having the half-brother making this decision, when he was not close to my friend and had a financial interest in his dying before his medical care exhausted his legacy. But, second, that in this culture where the whole process of dying is medicalized and people often spend their last months in hospitals and in pain, accelerated death of the patient isn't always the worst outcome.
My conclusion is that there are no universal solutions, and the less the state has to do with this process, the better. But that traditional ways in which loved ones make life-or-death decisions are preferable to institutions that either keep the patient alive at all costs or make a bureaucratic decision to kill him.
That seems like a fair and balanced assessment. This is a complex issue, but I agree that "the state should have no role in deciding if or when to terminate someone's life."
